A service plan is customized for each person according to his or her own needs. Because the aging process is different for each individual, we monitor our residents daily to identify any potential health changes. Over time, our staff members get to know our residents very well, and together we create a safe environment and a life filled with enriching experiences.

Congregational Home's new Medicare-certified Short-term Rehabilitation Unit provides private deluxe accommodations combined with caring nursing staff and therapists.

Therapy options:

Physical Therapy - Helps individuals address problems with pain, balance, walking and lower body strength. One of the primary goals of physical therapy is to prevent falls.

Occupational Therapy - Concentrates on issue of self-care including personal hygiene, dressing and performance of daily living functions such as meal preparation and laundry. OTs also work with individuals who are experiencing pain or decreased function in their arms or hands. Adaptive equipment may be introduced to make everyday activities safer and easier.

Speech Therapy - Focuses on communication issues with individuals who are experiencing problems with memory, word-finding, understanding, and problem solving. Speech Pathologists are also key to assisting individuals who are experiencing swallowing difficulties.

Skilled Care at Congregational Home is designed for long-term residents having advanced limitations in self-care. These residents regularly require a high level of assistance and medical management. Our Skilled area consists primarily of single occupancy rooms with private bathrooms with walk-in showers, but it also includes several two-room suites that offer the luxury of a separate living room and bedroom with private bath. A schedule of weekly activities provide a variety of life-enriching programs for the overall well-being of our residents.

Our available Memory Care is designed to increase comfort and reduce anxiety for those residents with Alzheimer's disease and other forms of dementia. Our wellness philosophy and personalized approach puts the resident first and nurtures the health and well-being of the whole person - mind,body and spirit.

Respite Care services are offered for individuals requiring short-term stays while caregivers are away from home or in need of a break from the daily routine, challenges and stress of caring for an aging adult.Respite Care has the benefit of renewing a caregiver's energy and outlook and is often referred to as the gift of time.
