The Gardens at Osage Terrace

The Gardens at Osage Terrace is a wonderful assisted living community in Bentonville, Arkansas. It is within fantastic Benton County which is part of the Natural State.

The Community Development Corporation of Bentonville/Bella Vista determined the need for an alternative housing for those residents that required additional assistance to remain independent, but were not ready to enter the long term care system.

The Gardens at Osage Terrace is committed to cultivating a community to allow choices that foster freedom from dependence and nurtures individuality for an aging population.  By creating a cultural change in the affordability and quality of care it will direct a higher standard for others to follow us in coming home

At Gardens at Osage Terrace  we create warm, supportive, home-like residences for older adults. From social, cultural and educational life enrichment programs to amenities that foster an easy way of life, our respect for each resident's individuality is at the center of all that we do.

Potential residents are assessed with a basic medical assessment tool.  Questions concerning various areas of care and activities of daily living are addressed.  Each area is assigned a point system.  The total number of points assigned indicates the resident's service needs and the level of care.  The Assisted Living II facility acknowledges 4 levels of care or Tiers.  Applicants without the need for healthcare services shall not be admitted to the Assisted Living II facility.  

We are very proud of the number and quality of our professional staff. We have very low turn-over so care is consistent and given cheerfully.

Medical supervision is accomplished with Nurse Oversight, licensed practical nursing 16 hours daily, and certified nursing assistants per shift. Licensed nursing administers medications. Residents are assisted with activities of daily living by nursing assistants.

A facility Medical Director, specializing in geriatric healthcare is available to see the residents of the facility, in-house to eliminate the need for outside transportation.

Service Care Plans are developed and monitored on a quarterly basis. Complete medical records are maintained with physician’s orders for placement, medication regimes and medical interventions.

Three balanced, nutrition meals are prepared and served daily in the facility dining room. A Registered Dietician oversees all physician’s ordered diets and special needs.

The current trends shows a higher success rate in holding smaller activity groups in addition to offering a variety of activities based on the level of function of the current neighbor.  Activities are intended to promote socialization, quality of life and strengthening physical, mental and emotional well-being.

Take a moment for a hot cappuccino, coffee or tea at our Bistro.  Drinks and snacks are always available.  The area provides a place to have a sip and chat, use our wireless Internet or watch the latest news on TV.  The Bistro provides a place for small gatherings and nook to visit with friends.
