Our Homes are conveniently located in Peoria and Surprise. They are all on .5 to 1 acre lots. They are all fenced and secured to allow safety for the residents while relaxing outdoors. They are secured homes with key pad locks, to prevent wandering and elopement. Security cameras are in the common areas to provide additional safety.
Five Star by elite offers the highest level of care available. Our homes are owned and managed by a PhD/DHA, healthcare administrator. We also have a full-time registered nurse and physical therapist and a licensed professional counselor. We are interfaced with the Pharmacy and all medications are filled and set up by the doctor and pharmacist. All Medication Administration Records are completed on-line and reviewed by the doctor and pharmacy.We provide Live entertainment and daily activities.
