Heritage Home Care, Inc. established in 2008, Heritage Senior Home Care, Inc. in 2012 and Heritage Residential Home Care, Inc. in 2018, are three residential care facilities for the elderly licensed by the Social Services and Community Care Licensing Department. We offer three assisted living, board and care homes for senior residents in California in the City of La Verne and Claremont. All homes are located in cul de sacs with quiet, safe and friendly neighborhoods.

Our goal is to help our clients achieve the best quality of life possible. We understand that one solution does not fit every situation so we take the time to listen and learn about each client's needs and preferences. This enables us to tailor a care program that maximizes the client's independence and dignity.

Physical wellness is our top priority. We know that residents have different health needs. That is why it is essential for us to work with family and physicians to create a health plan that is suitable to their needs. We strive to keep our residents engaged in life on a personalized level by helping them continue activities related to their hobbies and interests.

We take pride in the quality of service we provide. Our caregivers are carefully selected and trained by an experienced registered nurse to ensure high level of personal care. Our home offers a relaxing, safe and friendly environment. We are committed to making our residents feel that they are a part of our family and be their home away from home.

We offer the following services: 

• 24-hour premier care and supervision
• Nutritional meals and snacks
• Special diet, if physician prescribed (additional fees may apply)
• Short-term and long-term care
• Dementia and hospice care
• Assistance with Activities of Daily Living (ADLs) - bathing, grooming, eating, etc.
• Transportation for doctor appointments, family visits, social
• events and other personal errands (rates & fees apply)
• Daily room cleaning, sanitation and laundry services
• Surgery recovery and companionship
• Bedside and incontinence assistance
• Medication management
• Ambulation and mobility assistance

We have indoor and outdoor activities that promote health and happiness of our residents. We take them to trips to the park, mall, restaurants, movies and other events. We encourage them to sign-up for community center activities like sewing, bingo, dancing nights and other social programs. We also offer stimulating activities at home such as barbecues, movie, music an game nights. Celebrating our residents special events is important for us. We welcome family and friends to visit and participate in birthdays, anniversaries and holiday parties.
