Monument Park Manor is a Residential Care Facility for the Elderly, located in the City of Perris, CA. We provide care to elderly clients who may have physical and medical challenges that requires care and assistance on their activities of daily living. Monument Park Manor staff are available to provide direct care services and supervision on residents ADL's or Activities of Daily Living. Services offered:

• 24 hour care and night supervison
• Protective supervision for mild to moderate alteration in mental status
• Assistance and safety supervision on mobility and transferring with the use of assistive aid
• Nutritious meal preparation
• Monitoring, management and assistance with medication
• Assistance with bathing, dressing and grooming
• Total incontinent care
• Housekeeping and laundry services
• Daily meaningful activities
• Coordinating with residents Medical and Dental provider, pharmacy
• Assistance with transportation arrangements
