AgapeCare is incorporated as an s-corporation established under the laws of the state of California. The agency is composed of management and office personnel, qualified and licensed clinicians, non-licensed caregivers, headed by the Administrator and Director of Nurses. PAS operate under it's own Policy & Procedure. Our company name is based on the key term Agape which means unconditional love and it is the highest form of love from a Christian definition and from that our guiding philosophy stands that we will provide an unconditional care in the comfort of our patient's home. We believe that aside from delivering quality care, we need to meet every patient's total well-being.

Home Health follows a physician's home health order, and is a level of service provided by licensed medical professional that includes:

• Home Health Aides assist with activities such as bathing and dressing.
• Skilled Nursing - Registered Nurses (RN's) and Licensed Vocational Nurses (LVN's), work with your doctor to provide you skilled nursing services.
• Physical Therapists & Occupational Therapists will come to your home to provide therapy such as exercise, gait training, and instruction on the use of durable medical equipment.

Using a team approach, we will work with you and your doctor to develop a care plan that will help you reach your optimal level of independence.

AgapeCare Personal Assistance Services provides a variety of personalized non-medical assistance and services to help out clients or their love one's maintain and regain your independence in the comfort of their homes.

These service include:

• Assistance with hobbies and pastime/recreational activities
• Transportation/escort
• Accompaniment to doctor appointments
• Respite and family relief
• Light Housekeeping
• Grocery shopping/errands
• Meal preparation including special diets
• Laundry
• Medication reminder
• Transferring
• Ambulation assistance
• Bathing/dressing assistance
• Oral hygiene and personal care services

AgapeCare Home Health provides a suite of services that work cohesively to help you in your recovery; for you to regain your independence and well-being faster.

Part of the goal is to help you learn about your disease or condition, medications, potential complications, and the importance of being involved in your own care. Our team works with your doctor to give you the health care that’s right for you in your home.

Our Services may include but not limited to the following:

• Medical Social Services
• Personal Assistance – part time to 24 hours x 7 days (Private Pay only)
• Diabetic Care
• Wound Care
• Teaching and observation of many diseases such as hypertension, coronary heart disease, or congestive heart failure, and etc.
• Feeding Tubes, Ostomy & Catheter Care
• Medication administration and teaching
• Exercise and diet
