Heatherwood is a privately owned memory care facility dedicated specifically for resident’s who are living with Alzheimer’s and Dementia related cognitive challenges.

Nestled in the heart of downtown Walnut Creek, Heatherwood Memory Care allows for a beautiful home-like setting for our residents as well as a vast array of shopping and exquisite culinary dining options only blocks away for our resident’s, their families and guests.

Each of our spacious private and semi-private rooms have direct access to our secure courtyard. Our community offers the tranquility of the outdoors yet provides a safe and secure environment.

Memory loss presents a unique set of challenges for, not only the resident, but their loved ones as well. Heatherwood Memory Care offers an opportunity for families to remove the demanding daily care routines off of themselves and onto us.

Heatherwood Memory Care is owner-operated in order to maintain a close-knit relationship between residents, their loved ones, and our staff. We strive to create a healthy, home-like atmosphere to allow our residents to enjoy comfort, fulfillment and security.

At Heatherwood we take pride in making the transition into our community as comfortable as possible. We believe that taking the time to understand the needs of our families is truly what sets us apart from other communities. Upon meeting with our experienced staff we will ensure that the process is both informative and effortless.

Our goal is to maintain a sense of individuality for each of our residents, all the while keeping a socially connected community. Each individual care plan is based on the specific needs of the resident, in which we will always incorporate personal aspects of a resident’s life, like their hobbies and interests, in an attempt to promote a continued sense of purpose, while being able to capture ongoing joyful experiences.

Our Memory Care program is constantly evolving and developing in order to best serve our residents. Our hope is that our program encourages personal choice and independence. Residents and their families are encouraged and supported to personalize their living spaces in both our shared and private room settings, so they can truly call Heatherwood Memory Care their home.

We provide kind and compassionate assistance to our residents, because we understand the fear and frustration brought on by memory loss. We have experience with varying stages of Alzheimer’s disease and Dementia and our caring staff is trained to support each resident as they continue their life journey.

Our Resident Care Director is a licensed nurse and conducts a comprehensive assessment for residents prior to their arrival at Heatherwood Memory Care. This assessment takes their physical, emotional, and functional status into account as well as detailed information regarding their background prior to joining our community. Our assessment, along with continual contact with each resident’s primary care physician allows us to develop an individual-based care plan for our residents.

Residents go through stages of Dementia and Alzheimer’s disease at varying stages and with different intensities. We are devoted to stay up to date with all changes to our residents needs. With our computerized care tracking system, we are able to update any necessary changes with our residents from behavior to modifications in their care plans, required assistance with activities of daily living, and changes in medications. All updates are made in real-time, in order to keep all staff up to date with the latest information on all of our residents.
