Guided by a philosophy that respects each person’s dignity and humanity, LiveWell supports people living with dementia and those who care about and for them.
LiveWell Navigators are available to guide you through life with dementia – whether you or someone you care for has been newly diagnosed or are experiencing the challenges associated with the disease progression. Navigation begins with a call or meeting to learn about your situation and needs. Then we work together to clarify your next steps and begin putting a plan in place. Through our work we have identified five LiveWell Pillars that support people and families impacted by dementia. Through individualized meetings, education and planning sessions, you and your Navigator will build a foundational understanding of ‘Me and My Story’ while exploring the five LiveWell Pillars: Understanding Dementia & Adaptive Strategies, Social & Meaningful Engagement, Emotional Support, Health & Wellness and Planning.
