West Forrest Personal Care Home was established in 2005 in quiet, beautiful , historic district of Harlem, Ga. 30814. We are licensed by the State of Georgia Health Care Facility Regulations and are inspected at random. We are Medicaid Providers for two State Medicaid Programs Community Care Service Program and Source Program . We accept Medicaid clients who are on either of these Programs and we also accept Private Pay and Long Term Care Insurance. We are licensed for the capacity of 12 clients Private and semi- private.
The following Services are provided: 24 hours 7 days a week supervision, Cable TV, 3 Hot Nutritious Meals, 2 Snacks, Assistance with showers, Dressing and Grooming, Laundry, Bed Linens , Rooms cleaned, Administering of all Medications, Handling all refills on Medications , Medications are delivered by Moye Pharmacy, All Medications are kept behind lock and key, RN Visits, LPN Vistits, and Doctor Visits, All staff is certified, we communicate closely with Doctors and family to make sure the clients is receiving the best of care and needs are being met.
