Madonna Heritage is a licensed, family owned, and operated assisted living home located in Jarrettsville, Maryland. Our home provides 16 beds for full-time and respite care services for you or your loved one. Because of our small size, we can maintain quality and provide individualized personal care, privacy, and independence for our guests/patients.
We are proud to offer three levels of care to accommodate the ever-changing health needs of our patients. We provide respite care when space is available. We are happy to assist in the arrangement of hospice services for end of life care.
Madonna Heritage has been providing comprehensive and personalized care for your loved ones since 1986. We provide our residents with a comfortable home setting, compassionate staff members, and a beautiful courtyard that overlooks our 14-acre property in Jarrettsville, Maryland. Our facility is located just minutes from Ladew Topiary Gardens and the scenic horse country of My Lady’s Manor in Monkton, Maryland. Our residents enjoy the site and sounds of nature in a safe and secure setting.
Level I Assisted Care
- includes all of the following:
- Choice of available residences, including all amenities and use of community spaces
- Individualized service and care plan developed by an interdisciplinary team, in conjunction with your physician that provides a holistic approach, and recognizes changes in condition
- Personal care from highly-trained staff available 24 hours a day
- 45 day physical assessment and medication review by delegating registered nurse
- All utilities included, private telephone and cable TV are not included. Communal phone and television available for residents
- Private suites available when vacant
- Routine maintenance services and upkeep of building and grounds
- Medication administration
- Assistance with preparations for dressing, grooming, and bathing
- Occasional reminders for dining and activities
- Monitoring of nutritional needs
- Reminders for self-managed incontinence
- Personalized care management by staff 24 hours per day in conjunction with your physician
- 24-hour security and emergency response system
- Three appetizing and nutritionally balanced meals each day plus healthy snacks
- Regularly scheduled spiritual programs
Level II Intermediate Care
Includes all Level I services plus:
- Moderate physical assistance with dressing and grooming
- Physical assistance with bathing
- Daily escort service to meals and activities
- Assistance with selection and set-up of meals as needed
- Physical assistance with and management of occasional incontinence
- Housekeeping, laundry, and linen service
Level III Total Care
Includes all Level I & II services plus:
- Physical assistance with dressing and grooming daily
- Physical assistance with showers or baths up to seven times per week
- Frequent care for incontinence, including additional housekeeping, laundry, and linen service
- More frequent nurse interventions and additional assistance with respiratory and wound care to the extent permitted by state regulations
- Regular nighttime safety checks as needed
- Two-person transfer assistance
