Autumn Hills Assisted Living is a facility which offers assistance with all activities of daily living. We house up to 16 residents and are located in beautiful Bemidji, Minnesota.
Our Assisted Living facility is able to provide a high level of care while increasing the personal satisfaction of the client at a reasonable cost to the consumer. Autumn Hills is licensed by the Minnesota Department of Health.
Autumn Hills Assisted Living is staffed 24 hours a day by trained Home Health Aides who are supervised by a Registered Nurse. We are locally owned and operated by Shelley Cloose, RN. All rooms are private and the entire building is handicap accessible. There are four bathrooms; two with showers and one with a handicap bathtub. There is a large dining room and living area for gatherings and activities. The back yard offers a cement patio with lots of seating and a gazebo.
Community members who care for a senior at home need to remember to care for themselves as well. For this reason, we will provide care and supervision for your loved one if you need to be out of town or cannot provide the care for any reason. Short-term and Respite Care are available from 1-30 days. All of the above services are included in the daily cost.
- Assistance with all aspects of daily living - dressing, bathing, grooming and toileting
- 3 nutritious meals per day plus snacks

- Medication Administration including insulin
- Assistance managing medical conditions such as diabetes and coumadin therapy
- Arrangement of medical appointments and transportation

- Socialization, activities and supervision
- Nurse call pendants
- Registered Nurse services available 24 hours a day
- Coordination with your physician, hospice or home care services
