The Memory Care Residences at Creekstone

The Memory Care Residences at Creekstone is a superb assisted living community in Fairport, New York. It is within scenic Monroe County which is part of the Empire State.

We provide a loving and joyful environment where Elders are cared for as family and can experience wellbeing while living life to its fullest. We believe it is our obligation to accommodate the health and personal care needs of our Elders and equally important to nurture their human spirit. Through our mindful and compassionate approach, we inspire our residents to live a happy and dignified life.

Our vibrant community specializes in the care of persons with dementia. At Creekstone, there are no long corridors, imposing medical equipment or medication carts. Instead, you walk into a ranch-style family home with an accessible kitchen, large living/dining area and 14 bedrooms! The front porch and secure courtyard allow Elders the freedom to enjoy the outdoors.

The Elders and staff of each residence become family and all help to care for each other and the home. The traditional hierarchy, common in long term care, is truly flattened with the resources and authority shifted from administrative structures toward those staff working closest with the Elders.

Shahbazim are the key staff members of each residence. The Shahbazim assist Elders with their personal care and offer opportunities for Elders to connect and build relationships. They also make the meals and maintain the home.

Residence Guides include the administrator and two nurse/care-managers provide clinical support to the family of Elders in each of the homes.

Our experience has shown us that this model is particularly comforting for persons living with dementia. The small, home setting is familiar and reassuring. More importantly, the Eden Alternative philosophy embraced by all who work in the homes, assures that the Elders are honored, respected and loved as family.

Memory loss is most often progressive and an Elder’s abilities may increasingly be compromised. At Creekstone, it is our hope to care for each Elder through the end of life. Our Enhanced Care Residence will accommodate the needs of persons living with advanced dementia with additional staff support. We also are able to partner with a home care agency to provide hospice care when the need arises.

Our respite program provides temporary care – from 1 week to 6 weeks – and is ideal for Elders who need transitional support after an illness. It also allows caregivers to travel, focus on family issues, or just take a much-needed break. Many respite guests enjoy their stay so much they become permanent assisted living residents.

We firmly believe that it is our  obligation to, not only, care for the health and personal care needs of our Elders but equally important, it is our mission to make their lives meaningful and fulfilling.  We work tirelessly to nurture the human spirit and alleviate the plagues of loneliness, helplessness and boredom.

At each of our three Eden Alternative residences, we offer leisure activities tailored to the unique needs and preferences of each Eden family. A primary goal is to foster meaningful relationships and create a shared experience of joy.

Elders may assist with the care of our pets or may participate in helpful roles within the household. Some enjoy working in our gardens and others take pride in giving the  blessing at meals.  These “family roles” provide purpose and alleviate the feelings of helplessness.

Our Enrichment Guides plan a daily calendar of activities focused on nurturing physical, social and spiritual well-being. Our chaplain offers 1:1 pastoral visits as well as worship services at the Homestead and Cottage Grove.
