Dutchess Care Assisted Living Facility

PAZ currently owns and operates three assisted living facilities in New York State. Each facility is serviced by Golden Care Licensed Home Care Services Agency (LHCSA), a PAZ affiliated LHCSA, which ensures continuity of care and communication throughout the service provider network. Licensed by the New York State Department of Health (NYS DOH), each PAZ facility is unique and tailored to the specific needs of its resident populations. Facilities are inspected regularly by the NYS DOH and PAZ is proud to have consistently maintained a record of substantial compliance through its dedication to quality assurance and person-centered care.

Each facility accepts private funds, long term care insurance and Medicaid (through the Assisted Living Program) for eligible applicants. All residents are treated with empathy, dignity and respect by professionally trained and certified staff. The assisted living facilities include:

Avalon Assisted Living

Built by PAZ in 2001, The Avalon is a 76-bed assisted living facility in Wappingers Falls, NY, where residents are treated with empathy, dignity and respect by trained and certified staff. PAZ Health Care is proud to offer an all-inclusive pricing model which allows families to make safe financial plans when caring for their relatives.

Dutchess Care Assisted Living

Built in 1974, Dutchess Care is a 92-bed assisted living facility in Poughkeepsie, NY. In 1981, PAZ took over the property and increased staffing levels to support the residents. Continuing its legacy of providing pioneering care, Dutchess Care is currently working with the NYS Dept of Health on a unique PILOT program within the existing Assisted Living Program to provide enhanced staffing levels and service adults with Traumatic Brain Injuries and other neurobehavioral disorders.

Crestview Manor Assisted Living

Crestview Manor is a 76-bed assisted living facility located in the hamlet of Hawthorne, in Westchester County, NY. This facility is proud to offer an all-inclusive pricing model which allows families to make safe financial plans when caring for their relatives. Built in 1974, PAZ took over the property in 1993, renovated the building and increased staffing levels to support the residents.

PAZ Management’s highly qualified staff members have been thoroughly screened, professionally trained and are closely supervised by a caring management team. Trained to recognize that each client has a unique background, diverse experiences and distinct goals, PAZ staff treat each client with empathy, dignity and respect.

At PAZ, clients are treated with the same courtesy, care and attention that they would expect from their own family.

PAZ Health Care provides the following services:

• Adult social day care program
• Community Licensed Home Care Services Agency (LHCSA) services, including: Nursing, home health aide and personal care aide services

Assisted living services, including but not limited to:

• Senior-friendly housing with heat, electric and utilities included
• 24-hour staffing
• Full meal services (including 3 full meals plus snacks each day, planned under the direction of a registered dietician)
• Social and recreational activities
• Nursing, home health aide and personal care aide services for assistance with daily living activities
• Medication management assistance
• Housekeeping services
• Laundry services
• Ongoing case management
• Coordination of professional (medical) services
