We are a small community with private and semi-private rooms available. All meals and activities are included with our affordable rates. Since we strive on quality care, our rates are not based on needs; rather we base them on the room you choose. All-inclusive rates include personal care items, medication administration, cable, telephone, and peace of mind with nursing staff on-site
Our homes are small by design with only 20 residents living in each building. The smaller community atmosphere allows our care to be the main focus as well as allowing freedom you may not receive at large institutions.
Skilled services provided in the comfort of your home, assisted living facility, or personal care home. Our teams of Highly Skilled Licensed Professionals make visits to your bedside. For individuals that meet medical necessity, home care services are a wonderful option to keep you safe, healthy, and independent. Services are covered by all Major Insurance Companies and Medicare/Medicaid. Our group will work closely with your physicians to develop the most comprehensive care plan to address all your needs. Skilled Nursing
Skilled nursing is a service that can only be offered by licensed nurses. This treatment is a higher lever of care. Skilled nursing includes an array of many complex services including Rehabilitation, would and dressing changes, ostomy and ileostomy care and an ever changing health status.
Whether you are receiving our services at home or staying with us at At Home At Parkland we can easily provide most all Skilled Nursing services.
Physical Therapy
Physical Therapy (PT) is a service that only be given by a licensed therapist. The aim of this service is to reduce pain and improve and restore mobility.
Our therapists will setup a full comprehensive care plan to help monitor and fully optimize your care.
At Home Health Care will bring this service right to your door. Whether you are living independently or here with us at At Home At Parkland.
Occupational Therapy
Occupational Therapy is a service that can only be administered by a licensed therapist.
The purpose for this therapy is to help develop, recover and maintain meaningful every day to day activities.
Our therapists will develop your own personal care plan to aid in your recovery.
We will bring this service right to your door, whether you are living independently or living here with us at At Home At Parkland.
Speech Therapy
Speech Therapy is a service that can only be offered by a licensed therapist. The purpose of this therapy is to help patients with speech, language, cognitive-communication and swallowing disorders.
Our therapist with formulate a full care plan for your recovery and maintenance. We will bring this service right to your home living independently or living here with us at At Home at Parkland.
Medical Social Work
Monitors patient condition by observing physical and mental condition, intake and output, and exercise. Assists patients by providing personal services, such as, bathing, dressing, and grooming. Aide services can be covered by some insurance policies or can be set up as a private pay program.
Home Health Aides
