KeystoneCare is a charitable nonprofit organization that provides a full continuum of home health and hospice services. For more than 20 years, we have been helping improve the quality of life for our patients and their families, regardless of stage of life or state of health. Our services include home care, palliative care, hospice, private duty, and Keystone InTouch telehealth.

Our services include (but are not limited to) post-hospitalization home care, palliative care, comprehensive hospice services, private duty care and telehealth services. KeystoneCare can provide home care, palliative care, and hospice care at assisted living and skilled nursing facilities, in patients’ homes, or at Keystone House, our 19 bed inpatient/residential hospice care facility staffed with kind and compassionate, skilled, and experienced providers.

With the highest level of comprehensive care available, our home care team works with patients, their families, and physicians to create a cohesive in-home care setting that promotes health and healing. Theses services are available:

• Post-hospitalization care
• Help managing chronic illness
• Assistance with daily activities
• Medication monitoring, adherence, and changes
• Physical rehabilitation
• Wound care
• Patient education on disease and self-care

Palliative care is appropriate at any age and for any stage of an illness and can be provided at the same time as curative treatment. Palliative care is health care that focuses on managing the pain, symptoms, and the stress of individuals and families facing complex, life-limiting or life-threatening illness. In order to improve the quality of life for patients and their families our interdisciplinary team may include:
