Stone Brook Assisted Living is for senior adults who wish to have as much independence as possible but will also need some additional assistance as well.
Stone Brook Memory Care is for senior adults with Alzheimer’s or related types of dementia who require a secure environment and supervision and assistance with activities of daily living.
Our community offers a safe, secure, home-like environment. We tailor our services to meet each individual’s needs including nutrition, incontinence care, personalized care, medication management, transportation, laundry and housekeeping services, and daily activities. We also offer respite care services for the home caregiver who may need a break for a day, a week, a month or possibly longer. Here at Stone Brook our residents build close friendships with new neighbors and enjoy a surrogate family in the caring team that is available to assist them with their day-to-day needs.
Stone Brook meets the individual needs of our residents while maintaining the highest level of choice, dignity, privacy, and respect. We have an easygoing, home-like atmosphere. Our devoted team is here to support our residents with all of their daily activity needs. The resident and family participate in planning the individual services that are intended to enhance daily life and independence.
