Sandy Ridge Assisted Living Apartments is a sensational assisted living community in Colfax, Wisconsin. It is within picturesque Dunn County which is part of the Badger State.
This new complex offers 10 single occupancy and 2 double occupancy apartments that are designed to provide care and support services to residents living independently.
Services include meals, medication management, housekeeping/laundry services and assistance with bathing. Each apartment has an emergency call system and staff is available to provide as needed assistance 24 hours per day.
A Sandy Ridge Assisted Living Apartment resident shall:
- Be 60 years of age or older;
- Be able to accept risk and sign an agreement;
- Be able to make health care decisions or share an apartment with a competent spouse or person who has legal responsibility;
- Be capable of recognizing danger, summoning assistance and expressing need;
- Be generally alert and oriented to time, place and persons;
- Be capable of acceptable interaction with others without disruptive or combative behaviors;
- Not require more than 28 hours of personal, supportive or nursing services in a week;
- Not have a medical condition(s) that requires the immediate availability of a nurse twenty-four (24) hours a day (i.e.; dispensing sliding scale insulin)
- Be able to move about safely with or without assistive devices such as canes, walkers, wheelchairs, etc.
- Be able to assist in transfer (does not need two-person transfers or the use of a mechanical lift for transfer);
- Be able to follow the house rules, policies and procedures such as, but not limited to, no- smoking in the facility;
- Be able to eat without constant supervision or extensive assistance;
- Be able to manage or accept assistance with incontinence;
- Provide to the facility evidence of financial viability for all assisted living costs;
- Allow the facility to conduct a comprehensive pre-admission assessment.
In order to maintain residency at Sandy Ridge Assisted Living Apartments a resident must:
- Remain capable of recognizing danger, summoning assistance, expressing needs and making care decisions. In addition, a resident must continue to be able to understand their need for supportive, personal or nursing services and must be able to choose services and understand the outcomes of their choices. Sandy Ridge Assisted Living will not retain residents who have an activated Health Care Power of Attorney or a Legal Guardian unless the individual is living in the same apartment with a competent spouse or designee.
- Continue to be generally alert and oriented to time, place and person. Sandy Ridge Assisted Living will not retain residents who have exhibited exit seeking or intrusive wandering behaviors.
- Continue to acceptably interact with others. Sandy Ridge Assisted Living will not retain residents who have exhibited disruptive or combative behaviors or inappropriate actions.
- Remain capable of moving about safely with or without assistive devices such as canes, walkers, wheelchairs, and so forth. Sandy Ridge Assisted Living will not retain residents who need two-person transfers or the use of a mechanical lift for transfers.
- Not have a medical condition(s) that requires the immediate availability of a nurse twenty-four (24) hours a day.
- Continue to be able to provide for personal needs, including dressing, bathing, grooming, toileting, etc. with assistance and within the maximum twenty-eight (28) hours of care per week provided by Sandy Ridge Assisted Living personnel and within the maximum number of hours of care per week that can be provided by outside provider from approved outside providers. This includes personal, supportive and nursing services. Sandy Ridge Assisted Living will not retain residents who require constant supervision or extensive assistance with eating. A resident must continue to be able to manage or accept assistance with incontinence.
- Maintain compliance with policies and procedures as outlined in the Resident Handbook and other communications.
- Maintain compliance with his or her Risk Agreement and Individual Service Agreement.
- Be willing to revise his or her risk Agreement and Individual Service Agreement when requested.
- Maintain compliance with Sandy Ridge Assisted Living policies relating to outside providers, and with any applicable outside provider agreement(s).
- Be willing to participate in reviews of his or her comprehensive assessment.
- Not exceed the level of service available at Sandy Ridge Assisted Living.
- Continue to pay the monthly fee and optional service fee(s) when due.
- Not pose an immediate threat to the health or safety of self or others.
- Agree to cooperate in an examination by a physician or licensed psychologist of the resident's choosing to determine the resident's health or mental status for the purpose of determining the appropriateness of retention, as requested by Sandy Ridge Assisted Living in its sole discretion.
- Continue to qualify for residency under the regulations applicable to RCACs, Wisconsin Administrative Code HFS 89.
