An Innovative Care, is an Assisted Living provider located in Pleasant Prairie, WI. Our homes include two Adult Family Homes (AFH) and a Community-Based Residential Facility (CBRF) which are all in serene neighborhoods located minutes away from hospitals, community activity centers, and shopping centers. We are fully staffed 24/7 with trained and competent caregivers. Our model of care focuses on providing a nurturing home where we build strong relationships with every resident. Since every individual is unique, our programs are customized with an Individualized Service Plan (ISP) to help keep the quality of care consistent for all.
An Innovative Care serves residents who are ambulatory, semi-ambulatory or non-ambulatory and who may also be of advanced age, physically disabled, terminally ill, or diagnosed with Alzheimer’s or a related dementia. Each resident unit is furnished with a bed, dresser, closet/wardrobe, and has plenty of space for residents to personalize their room with belongings.
Our goal is to provide exceptional care and top notch services to our residents in a family-like environment that does not feel like an institution. We take pride in making sure each resident’s day is made brighter and easier. Resident services include:
- 24/7 assistance with ADL’s: such as bathing, grooming, housekeeping, & medication administration
- Three nutrutious meals a day and snacks. Special diets will be accommodated as prescribed by a physician and designed by a registered dietitian
- Daily Activities and Outings
- Social, recreational and educational programming
- Monitoring of changing personal and healthcare care needs
- Transportation to and from doctor appointments & assistance with other transportation arrangements
- Assistance in utilizing community resources
