Songbird Pond is a 44 bed assisted living community in Plymouth, WI serving the long term living needs of families in the East Central Wisconsin area. The community initially opened for the care of residents in 2008 and underwent a 100% expansion in 2018.
Songbird Pond offers several types of care coordinated by a RN Nurse Manager, in conjunction with the resident, family and physician.
Our services include complex health management including insulin dependent diabetes, medication management and administration, catheter care and wound care needs. Our professionally trained staff assists our residents with personal care needs, bathing and incontinence care.
Freshly prepared meals and snacks, housekeeping services, laundry service for personal clothing, linens and towels are all part of our “ just like home“ atmosphere.
Daily planned activities and regular community outings keep our residents as active as they desire and transportation to medical appointments is coordinated by our staff

Nestled next to a pond on three landscaped acres in southern Plymouth, WI, Songbird Pond offers its residents a quiet and comfortable place to call home. All rooms at Songbird Pond are private with private baths and walk in showers. We also have large spacious 2 room suites. We have several spacious common rooms where our residents participate in stimulating activities and enjoy entertainment.
