The Ohio Home Care Waiver allows individuals with physical disabilities and unstable medical conditions to receive care in their homes and communities instead of nursing facilities, hospitals or rehabilitation facilities.

How do I apply for Ohio Home Care Waiver?

To request an Ohio Home Care Waiver, complete the ODM 2399 waiver application and submit it to your local county department of job and family services. You may also need to complete a Medicaid application if you do not currently receive Medicaid coverage.
