Encanto Palms Assisted Living is a terrific assisted living community in Phoenix, Arizona. It is within alluring Maricopa County which is part of the Grand Canyon State.
Encanto Palms prides itself on the educational training, dedication, and continuity of our care staff. Each program unit is maintained by a Care Coordinator who oversees day-to-day clinical operation, handles staffing, and oversees implementation and follow-up of resident care. Our care givers are required to successfully complete ADHS-approved directed care training. In addition, they attend our orientation and training program focusing on approaches and interventions for dealing with low acuity behavior issues and working with people with dementia. All program staff are required to attend on-going continuing education training sessions.
Our Encanto Palms care team includes the unit coordinator, community LPN, primary care provider, behavioral health consultants, case managers, and caregivers. Each resident is reviewed by the care team within seven days of admission and a minimum of once a quarter thereafter.
At Encanto Palms we maintain two distinct and separate comprehensive/holistic programs for individuals exhibiting low acuity behavioral problems. One program for residents with dementia behaviors and the second program for residents with non-dementia behaviors.
Within our community, we offer three unique behavior programs that provide a highly structured and therapeutic approach to care that specializes in the management of behavior problems.
- Mirage is a secure program designed for persons diagnosed with dementia.
- Sahara is a secure program assisting persons whose behaviors are associated with diagnoses other than dementia.
- Oasis is an unsecured program for our cognitively clear residents who have demonstrated the ability to maintain their behaviors in an unsecured environment.
The Mirage and Sahara units provide a homelike environment for residents with low acuity behavioral issues requiring the stability, structure, and safety of a secure living environment. Two distinct and separate units; our Mirage Unit is home to residents diagnosed with dementia with our Sahara unit providing a home for persons whose behaviors are associated with diagnoses other than dementia.
Both units provide a therapeutic care program facilitated by a professional staff that are highly trained in working with persons with dementia. We place a special emphasis on effective approaches and interventions in dealing with low acuity behavior issues.
ncanto Palms maintains an activity program that meets the diverse behavioral, intellectual, emotional, social, and spiritual needs of all program residents. Our full-time activity director offers activity programming designed uniquely for each separate and distinct unit. All caregivers, care coordinators, and staff members are cross-trained to help facilitate activity programming through out the day, 7 days a week.
Relationships, continuity of care, and comfort are key in maintaining Encanto Palms’ family atmosphere. Both units offer comfortable semi-private and private rooms. Families and residents are encouraged to bring personal items and furniture to create a homelike atmosphere.
The Oasis unit offers a more independent residential option for residents who have demonstrated the capacity to manage behaviors and function effectively in a less restrictive environment. Our step-down solution from the Sahara unit, the Oasis unit provides both semi-private and private rooms.
