Maryland Gardens is a 100-bed, AHCA 2019 Silver Quality Awarded skilled nursing facility (nursing home) and assisted living facility. Our specialties are to provide residents who exhibit behaviors in either a dementia or non–dementia setting. We offer these services and others, in a warm, friendly park–like environment with orange groves and large grass areas.
Maryland Gardens Skilled Nursing Unit serves adults whose primary needs are for behavioral services, and who may also have secondary medical complications that require skilled nursing. Our Challenging Dementia Assisted Living Unit provides a safe, residential setting, with nutritious meals, life-enriching activities, and quality care in a warm and friendly atmosphere. We offer each resident courtyard access to enjoy green grass and fragrant trees.
The unique courtyard invites socializing within a secure, home-like environment nestled in prestigious north central Phoenix. Our secure Alzheimer’s residence provides a special program for our residents. Trained caregivers are on staff 24 hours a day. We assist with bathing, dressing, grooming, dispensing medications, and provide 3 meals a day plus snacks and more. Our activity program offers residents the opportunity to be actively engaged and involved in life’s daily pleasures. Emergency nursing is available 24 hours a day through our skilled nursing unit.

Maryland Gardens is a licensed Directed Care Assisted Living Unit, with a program specifically designed to provide a safe and supportive living environment for persons with dementia who exhibit challenging behaviors. Our specialized program will have a focus on these individuals. A multidisciplinary approach, high staff–to–resident ratio, an extensively trained staff, supportive environment, and targeted activity programming are just part of the aspects of the program.
