Patterson Care Home is a wonderful assisted living community in Patterson, California. It is within enjoyable Stanislaus County which is part of the Golden State.
Patterson Care Home, LLC is a family owned business that takes pride in giving compassionate and client centered care to our residents because we treat them like our family.
Our residents will get more focused care, time and attention through our two available caregivers during day time and one caregiver on night shift. The ratio is 3:1 (residents-caregiver).
Our care team is available 24/7 allowing our residents to fully feel secured, loved and well taken care of. Residents can also enjoy our secured outdoor patio where they can relax and savor the sunshine and fresh air.
We achieve our facility mission through the following
- Ensuring that facility staff meet on a minimum monthly basis to discuss residents need any potential changes in condition.
- Ensuring that the facility's food and services and dining program meets or exceeds industry standards and state regulations and offers a home-like dining atmosphere, considerate of individual residents likes and preferences.
- Ensuring that the facility external facade and internal decor is aesthetically comforting, home-like and free from hazards. The facility offers on site tours by appointment and prospective residents and/or family members are encourage to visit the facility prior to admission.
- Ensuring that staffing ratios are appropriate to the level of care of the residents
- Ensuring that we are proactive in dealing with prescriptions and doctor's orders so we can help our residents to receive a timely attention.
- Placing higher value on customer service and communication with residents and families.
- Ensuring that staff training requirements meet or exceed industry standard and state requirements to ensure they are knowledgeable in the areas impacting resident care and familiar with individual resident's needs.
- Ensuring that an activity program is in place and that activities are individualized to residents interest and appropriate for their physical/mental condition, level of participation and social preferences.
Our facility provides the following basic services:
- Twenty-four (24) hour care and supervision, seven (7) days per week, all year long.
- Private and semi private resident's bedrooms, which include comfortable and suitable bedroom furniture and home like furnishings. Residents may upon admission, provide their own personal furniture to utilize in their bedroom, or may use the option of utilizing the furniture provided by the facility.
- Resident have use of all common areas of the facility, unless prohibited by the residents physician due to illness.
- Maintenance of bedrooms, including the right of the licensee to charge the residents for repairs related to damage which is beyond normal wear and tear. Residents are not permitted to make any structural or physical changes to their bedroom without the sole permission and agreement of the licensee.
- Personal supplies, such as clean linens, blankets, bedspreads, sheets, pillow cases, mattress pads, bath towels, hand towels will be provided to each residents.
- Three (3) meals per day and in between meal snacks at least three (3) three times per day, or as needed.
- Modified diets are available upon request, as ordered by the resident's physician, and in consultation with a dietary consultant.
- Laundry service (does not include dry cleaning)
- Basic housekeeping, including cleaning of the resident's room at least once per week or more frequently, if needed.
- Assistance with showers and bathing, at least twice weekly, or more frequently, if needed.
- Assistance in arranging for transportation to and from medical and dental appointments. The responsible party or an approved representative may choose to provide their own transportation to the resident appointment. A planned activity program with assistance in arranging transportation to participate in community events or activities outside of the facility.
- Assistance with activities of daily living (ADLs), including but not limited to bathing, dressing, hygiene, grooming, eating, mobility tasks and incontinent care.
- Bedside care, including meal service for minor or temporary illnesses, or per resident's request.
- Daily assistance with taking medications as ordered by the resident's physicians, including prescription, non prescription and PRN medications.
- On-going observation of the resident to identify and report on any changes in condition. Changes in condition will be reported to the physicians, responsible party, and the Licensing agency, if applicable.
- Additional care and supervision as needed for residents with mild cognitive impairment and/or dementia, in accordance with the facility Plan of Operation, which is available for review upon request.
- Supportive services as determined by the resident, assessment, or by physician's order, or by any licensed medical professional.
- Basic cable or satellite service in each resident bedroom for additional charge and internet in the common living room area (Note : resident to provide the television for the bedroom) All other monthly utility charges such as gas, water, electricity, sewer are all included in the monthly charge.
