Our goal is to give the families confidence knowing their loved one is receiving the highest quality residential care. We provide a safe and nurturing environment where the needs of safety, nutrition, medications, hygiene, socialization & mental stimulation are a priority to our staff.

Our Services

• Facilities Operations are overseen by a Registered Nurse
• 24-hour care and supervision 7 days per week all year long
• Private Room or Shared room Based on availability
• At full Capacity Patient Care giver ratio is 3:1 with 1 Separate AWAKE staff during night
• hours
• Daily Activities (Morning workout, Balance Exercises, Bingo, Puzzles, Music Playing,
• Planting, Spa therapy, Pet therapy, and Memory games)
• Assistance with activities of daily living. Including but not limited to bathing, dressing,
• hygiene, grooming, eating, mobility tasks and incontinence care.
• Assistance with showers and bathing two times per week minimum.
• Daily assistance with taking medications as ordered by the resident’s physician,
• including prescription, non-prescription, and PRN medications
• Residents have use of all the common areas of the facility including areas like the family
• room and backyard patio whenever they desire.
• Three (3) meals per day will be provided with snacks in between each meal. Modified
• diets are available upon request or as ordered by the resident’s physician.
• The residents will be provided with personal supplies as followed, clean linens, blankets,
• bedspreads, sheets, pillowcases, pillows, mattress pad, bath towels, and hand towels.
• Cornerstone Senior home care will change these linens at least once per week or more
• often if needed.
• Laundry Service (does not include dry cleaning)
• Assistance in arranging for transportation to and from medical and dental appointments.
