Utopia Assisted Living Services is an attractive assisted living community in East Haven, Connecticut. It is within classy New Haven County which is part of the Constitution State.
Utopia Home Care's East Haven branch offers skilled nursing and therapy services, as well as home health aide (HHA) and companion care. Utopia Home Care's nursing supervisor, home health aide supervisor and community liaison often meet with individuals and families to arrange services for patients following a stay at local hospitals such as Yale New Haven Hospital, including Saint Raphael, Milford Hospital, and Griffin Hospital.
Registered Nurses (RNs) are an integral part of our home care team employed at every Utopia Home Care location. Our Medicare Certified branches in Connecticut rely on them to make skilled nursing visits as well as supervise home health aide services.
Typical Registered Nurse Tasks Include
- Development of nursing care plan
- Periodic assessment of patient condition and vitals
- Medication management and administration
- Wound care including dressing changes and tracking of healing progress
- Diabetes monitoring and management
- Supervision of home health aide activities
- Communication with physician for care plan or medication updates
- Care planning with Physical or Occupational Therapist for rehab services
- Make recommendations for new devices or tools to improve patient's quality of life
Home Health Aides (HHA's) assist your loved one with "Activities of Daily Living" (ADLs), but have additional training for clients requiring a higher level of care. These aides undergo additional hours of in-service training throughout the year and are regularly tested on their specialty care training.
Typical Home Health Aide Tasks Include:
- Meal preparation for special or complex diets (low sodium, low fat, etc.)
- Assisting with Range of Motion exercises
- Simple dressing changes
- Knowledge and training on the use of assistive devices, such as a Hoyer Lift
- Assisting with personal hygiene, such as toileting, bathing, dressing, and feeding
- Medication reminders
- Meal preparation
- Housekeeping and laundry services
- Assistance with walking, standing, sitting
As with all of our aides, HHAs are state licensed/certified, bonded, insured, and pass regular in-service exams. All staff undergo rigorous criminal background checks, as well as drug screenings prior to be placed within a client's home. All services and care plans are monitored routinely by our dedicated staff of RN Supervisors to ensure client safety and well-being.
Physical Therapists (PTs) are employed by our Medicare Certified branches in Connecticut. Physical Therapists have specialized training to assist clients with developmental or physical conditions that limit their mobility or range of motion. Treatment focuses on working towards regaining a normal level of movement for the client and preventing future loss of mobility.
Physical Therapists are state-licensed professionals and thoroughly screened prior to being hired by Utopia. Therapists assist in initial client evaluation for services, develop the plan of care, and providing skilled therapy services.
Typical Physical Therapist Tasks Include:
- Consulting with patient and nurse to assess physical condition and ailments
- Diagnosing movement dysfunction and developing a corresponding treatment plan
- Teaching patients therapeutic exercise techniques
- Providing stimulation and massage to promote healing and alleviate pain
- Assisting patients with the use of equipment including walkers, wheelchairs, and home devices
- Working with physician and nurse to track progress
- Advising patient on best in-home exercise and treatment options
