Brandywyne Health Care Center is a 120-bed skilled nursing and rehabilitation center located in Winter Haven, FL. Along with being the bridge between hospital and home, we offer a variety of services to promote motivation, self-expression, self-reliance, and independence. Our team of Physical, Speech and Occupational Therapists along with our skilled nursing staff will strive to make your stay comfortable and beneficial. We have a variety of activities and social functions to meet individuals mental and physical capabilities. We hope you consider us as you are making your discharge arrangements. We have different levels of care to meet your needs.
Our nursing staff consists of highly qualified RNs, LPNs, and CNAs that provide exceptional 24-hour care to all of our residents. We work in coordination with the physician to develop the best plan of care to meet the patient’s needs. This plan will be specific to the patient’s condition and goals. Our licensed nursing staff helps to manage and administer medications and assists with daily care needs. Our primary goal in short-term rehabilitation is to get patients back to their prior level of functioning.
We offer long-term care for our resident’s that have either chronic conditions or physical limitations that they are unable to manage at home as well as those who have memory impairments. We also offer end-of-life and Hospice Care. When our resident’s reach the final phase of their life, care emphasis is placed on comfort. In conjunction with local Hospice providers, our facility focuses on being responsive to the special needs of the resident and family during this time. Our primary goal in long-term care is to help people function at the highest level possible.
Brandywyne’s Rehab Team consists of in-house licensed therapists in the fields of physical, occupational and speech-language therapy. We also offers Home Assessments to residents at discharge to determine any recommendations for a safe transition to home.
