In August 2018, we opened our new state-of-the-art Care Center located in Asbury, IA. Our new facility was constructed to accommodate a variety of living options whether it be short-term or long-term. We provide Skilled Nursing Rehabilitation, Assisted Living, Memory Care-Assisted Living and Long‐term Nursing Care with Restorative Programming.

Our unique, inclusive approach creates the perfect environment for healing, and translates to a healthier, stronger you. Additionally, our extraordinary team of highly skilled and experienced nurses, therapists and staff are capable of delivering complex services for residents with many types of conditions such as stroke, fractures, fall injuries, post-surgery recovery and wound care.
After a surgery, illness or accident our immediate objective to our residents is to stabilize and strengthen them so that they can return to their independent lifestyle as soon as possible – stronger and with their spirits lifted. At Hawkeye Care Center , we provide leading edge, skilled therapy services including physical, occupational and speech therapy.
In addition to our short-term rehabilitation services, we are also focused on the needs of residents who require lengthier stays and have designed our community to ensure their comfort and well-being. Our long-term care is geared for individuals that may require 24 hour care, assistance with personal hygiene, medication management, meal preparation and/or assistance with activities of daily life.
Most importantly, residents will be in an environment where they feel at home and where our staff goes out of their way to get to know them personally. We are also skilled in assisting family members who are faced with the often difficult process of making choices for loved ones and can help with hospice and respite care services if needed.
Assisted Living at Heritage Suites offers a comfortable Studio with assistance available when needed. Put your worries away about housekeeping, meals & other chores. It’s all included at Heritage Suites. This community offers 14 assisted living Studios and is staffed with licensed nurses & caregivers, on call 24-hours a day.
Memory care-assisted living at Francis Suites is very similar to Heritage Suites except it offers a SECURED (delayed egress doors with keypad entries) living environment specifically designed for residents with Alzheimer’s or Alzheimer-like dementia. It is staffed with licensed nurses, on call 24-hours a day & specially trained staff that engage residents & encourage individual choices throughout the day. This gives the family a peace of mind that their loved one remains in an environment that preserves warmth, caring and the comforts and routines that help make a home.
