Beaver Dam Nursing & Rehab Center is a 81 bed long term care/rehab to home facility located in Beaver Dam, KY in a scenic country setting. We are owned by the Providence Health Groups and have been an operating nursing facility for over 40 years. The original structure was a two story home; but it has since been renovated to fit the needs of the numerous patients we have cared for over the years.

We consider our residents as part of our family and take pride in our facility and the care we provide.
