Sheldon Meadows Assisted Living Center is a superior assisted living facility, known for high-quality care and services as a Home for the Aged, licensed by the Michigan Department of Health and Human Services (MDHHS). We are located in Hudsonville, Michigan, and boast beautiful grounds and numerous conveniences for our residents.
The competent and caring staff at Sheldon Meadows is trained to provide residents with services including housekeeping, laundry, and daily meals, as well as to monitor residents’ mental, physical, and social health.
It is the mission of Sheldon Meadows Assisted Living Center to provide a home setting for the care of senior adults. We aim to create an environment with as few restrictions as possible for our residents, and to encourage their personal growth and happiness during their time with us. We strive to meet the needs of our residents fully, while respecting their independence and dignity.
Included in our basic monthly fee are the following personal care services, intended to provide residents with the standard of living they deserve and the assistance they need. Staff members are available around the clock to serve the needs of residents, planned or emergent.
Sheldon Meadows provides residents with healthy home-cooking everyday in the form of three scheduled meals and snacks that are available at any time. Snacks include fresh, homemade baked goods, ice cream, sandwiches, fruit, coffee, tea, juice, and more!
Breakfast is served from 7–9 a.m., hot and to order! If you’re not an early bird, don’t fret, cold breakfast items are available after 9 a.m. Lunch, the main event, occurs daily at noon. Supper happens between 5 p.m. and 6 p.m.
You can eat in the dining room with other members of the community—after all, eating is a social experience—or have food brought to your room (at an additional cost). Feel free to invite your friends and family to join you for meals, too (nominal fee applies).
If you would like us to administer your medication, remind you to take your medication, or open those pesky containers for you, we are happy to do so. We have a professional nurse on staff to supervise medication assistance activities.
Everyday we meet with our residents to give them their mail, and to make sure that they are well and happy.
Sheldon Meadows has an experienced and friendly maintenance staff to perform all necessary maintenance tasks and upkeep at our facility, from connecting the cable to fixing leaky faucets and more.
Additional services are available to supplement our service offerings, should you require them. Supplementary care may include
- Assistance bathing in resident’s shower or in hydrotherapy tub
- Complex medication administration and testing (insulin testing, etc.)
- Evening tuck-in
- Meal time assistance
- Assistance with toileting and incontinence management
- Assistance with dressing and grooming
- Escort to meals and activities
- These additional services may incur additional costs.
We conduct a Personal Needs Assessment to determine the level of support required by each resident. Assessment factors include daily living activities, mobility, cognition, and continence. At the conclusion of an assessment, Sheldon Meadows staff will meet with the resident and his or her family or representative to discuss the assessment, review any cost or rate changes, and develop an individualized service plan.
Assessments are made before move-in, 30 days after move-in, and quarterly (at minimum). Additional assessments may occur after acute episodes such as serious illness, injury, or hospitalization.
