Oak Terrace features spacious one and two bedroom, air-conditioned apartments ranging from 580 sq. feet to 1,200 sq. feet. Large studio apartments are also available.
Residents and families have the assurance of knowing that staff assistance is available 24 hours a day to provide personal care services to those residents who need help with daily activities. We employ full-time RNs, LPNs, CNAs, and a full-time Activities Director. Autumn Lane is not just a place for individuals who need memory care, our staff can meet the needs of those who feel more comfortable in a smaller setting, or who may require clinical care not able to be met in assisted living communities. Oak Terrace Health Care Center of Gaylord is one of the best skilled nursing home in southern Minnesota. We have centered our community around the residents we serve. We take admissions 7 days a week, 24 hours per day. We offer therapy services 7 days per week and have expanded amenities for our short-stay clients!
