Country Club Care Center

Country Club Care Center is a certified, privately owned care center that has offered residents optimal care since 1996. The community offers residential care services, long-term skilled nursing services, short-term skilled nursing services, rehabilitation services and highly specialized wound care services. The certified staff at Country Club Care Center work to ensure the safety, comfort and maximum well-being possible for each resident.

Our highly-trained teams of licensed professionals offer a full complement of health care and personal services including skilled nursing, physical therapy, occupational therapy, speech therapy, resident/family counseling, dentistry, podiatry, optometry and audiology. The competitive staff to resident ratio ensures that everyone receives a high level of individual attention.

We are mindful that moving to a new place may not always be easy. For this reason, we are proud to serve our residents with a full-time admissions coordinator. Our admissions coordinator visits with discharge planners, case managers, individuals and family members who are considering either a short or a long term stay at Country Club Care Center.
