Countryside Home is a premier assisted living community in Madison, Nebraska. It is within colorful Madison County which is part of the Cornhusker State.

Countryside Home provides many levels of care from slight assistance in our Assisted Living to 24 hour nursing care in our Skilled Nursing Facility. Our professional nursing staff, along with all other departments, provide quality care to our residents. Other departments include social services, dietary, physical, occupational, and speech therapies, activities and many more.
Our Assisted Living has its own dining room, solarium, sitting area, and laundry facility. There are 14 individual, one-room apartments with their own private baths. Our staff provides supervision from 6:00 AM to 10:00 PM and the Skilled Nursing Facility staff performs nightly rounds. Our Assisted Living offers a variety of activities, medication passing, laundry, and housekeeping services for our residents.
Countryside Home provides a place of care for individuals who require short term rehabilitation to recover from illness or surgery prior to returning home to live independently. Countryside Home also offers long term care for individuals who require ongoing nursing care. Our goal is to provide a home for our residents and ensure that they continue to live their life with quality and dignity.
Countryside Home offers physical, occupational, and speech therapy for our residents and members of the community. As a part of physical and occupational plans of care, we also offer phased progression aquatic therapy. Aquatic therapy provides a means of exercise that is often times safer and more comfortable than land-based exercise. The water temperature is maintained at 92 degrees, to aid in muscle relaxation and pain control.
The staff at Countryside Home will assist residents with care while attempting to maintain the highest possible level of independence. The residents are offered whirlpool baths or showers. The beauty shop is open two days a week and is a great place for residents to be pampered and to socialize. The Activities department provides nail care on Fridays, and the aides can assist residents during the remainder of the week by request. A podiatrist comes once a month.
The housekeeping staff does an outstanding job of maintaining a safe and clean environment for our residents. They also provide laundry services for both skilled nursing and assisted living residents. They assist residents and families with moving, decorating, arranging furniture, and putting away belongings. Their primary goal is to provide our residents with an environment they can be proud to call their home.
Countryside Home prides itself on its interdisciplinary staff of professionals who work together to monitor and maintain the residents’ health. In addition to the nursing staff, we have onsite therapists and are visited regularly by a dietician, pharmacists, general practitioners, a nurse practitioner specialized in psychiatric care and a podiatrist. We also offer transportation to and from medical appointments.
Countryside Home provides a safe and secure home for the residents. The residents are evaluated on a regular basis and a care plan is established to best meet their needs. The goal is to prevent falls, injuries and any other possible consequences of decreased safety. Entrances to the facility are locked from 9:00 PM to 5:00 AM.
Countryside Home’s nursing staff distributes medications to the resident as ordered by their physicians. Our consulting pharmacist evaluates the medication regime of each resident to ensure appropriateness of care. In Assisted Living, medication assistance is provided if necessary.
