Northbrook Heights is a community based adult home, providing room, meals, housekeeping, personal care, and 24-hour supervision in a warm, friendly, and quality living environment. Licensed by the New York State Department of Social Services, Northbrook Heights is associated with Assisted Living Program.
The Assisted Living Program (ALP) is a 56 bed facility which is licensed by the N.Y.S. Department of Social Services and the N.Y.S. Department of Health. The ALP is an entity established and operated for the purpose of providing long or short term residential care, room, board, supervision, and providing or arranging for home health services.
The Adult Care Facility (ACF) program is designed to provide monitoring and the care and services listed above to more independent higher functioning residents.
Our ALP program is the only New York State Licensed Assisted Living Program in Cayuga County. Residents in the ALP require and receive a higher level of care and services than in the ACF. The ALP level of care includes the development of an individualized care plan for each resident by a Registered Nurse for such assistance as transfer, ambulation, toileting, comfort checks , escorts to and from meals, and care and services for other special needs. Residents meeting income and financial resource criteria may have the cost of the ALP services covered by Medicaid. Health monitoring is provided daily by licensed nursing and social worker staff to determine residents’ needs. This includes communication with physicians and other health/medical care providers and arranging appointments, obtaining prescribed medications, obtaining urgent care or emergency care if needed, and directing personal care staff to meet the changing care needs of each resident.
Northbrook Heights offers an array of recreational activities on a daily basis including; bingo, exercises (including modified zumba), game night, happy hour, ice cream socials and a variety of entertainers. Additionally, periodic outings to places of interest are available for a nominal fee.
Northbrook Heights is fortunate to have an experienced chef who is involved in the preparation of three homemade meals each day. Residents are provided the opportunity to provide input into the development of our seasonal menus which is then reviewed and approved by a Registered Dietician. Additionally, residents are offered between meal snacks daily.
