S.E.M. Haven Health & Residential Care Center

S.E.M. Haven Health & Residential Care Center is a pleasing assisted living community in Milford, Ohio. It is within pleasing Clermont County which is part of the Buckeye State.

SEM Haven is a non-profit ministry providing nursing and residential care for older adults of varying economic, ethnic, and religious backgrounds. We strive to preserve and promote human dignity, self-determination, individual freedom and accustomed lifestyle.

Our vision is for SEM Haven to be a home where companionship and caring relationships thrive through empowerment and personal choice.

Our large nursing home and assisted living center can be compared to a neighborhood with eight homes (Households). Each Household has a living room, dining room, kitchen and resident bedrooms. Households are entered through a front door. The Elders who live in the Household dine there and participate in social events with the other Elders and staff. Our Households have consistent staffing and team leadership. Elders and their families are part of the decision making process about their lives and care.

Assisted Living Elders have a great home where they maintain maximum independence yet receive the help they need with medication administration and supervision or assistance with activities of daily living. The focus is on the individual and also on providing great social events and opportunities to interact with others.

• Nurses and care coordinators are on duty at all times to provide assistance that may include help with bathing, walking, dressing and other activities of daily living.
• Team based care empowers staff, Elders and families to work together to make decisions about life on the household.
• Monthly events include crafts, social activities, exercise, interaction with our household pets, gardening and bus excursions

At SEM Haven there are three Households dedicated to care of those living with Alzheimer’s or other dementias. We offer two levels of memory care (Assisted Living and Skilled Nursing) to meet each individual’s needs. We emphasize a positive physical and social environment with special activities programming to meet the needs of the Elders.

Our two Assisted Living households are small, just 8 or 9 Elders each. This allows for highly personal care in a quiet and comfortable setting.

• There is staff present 24 hours a day in each household along with a licensed nurse dedicated to assisted living.
• Continuity of care is achieved by having the same staff caring for Elders on a regular basis.
• Under the direction of a Clinical RN Nurse Leader, a separate household supports Elders with later stage dementia who require extensive nursing care.
• Our Skilled Nursing Memory Care Household is Canfield Court.

Our household model of care is a wonderful way of life for our Elders who, together with family members and staff, work as a team to make decisions about care and life. Each household has an RN clinical nurse leader and a team leader who work together to provide a positive and caring environment for our residents.

SEM Haven Rehab provides professional nursing and therapy care, usually following a hospitalization. Our therapists and nursing staff are specially trained to promote healing and strengthening by helping our rehab guests to achieve their goals and to return home.

Outside of the Household there are areas shared by all; Beauty Shop, Gathering Room (main activities center), Chapel, Therapy Gymnasium, Reception Area and Lucky’s Café. Members of every Household gather in these community areas and there is visiting among the Households as well. Parties and competitions are commonplace and include the Annual Senior prom, Olympics, Chili cook-off and sporting events.
