At The Bridges at Bent Creek, each resident receives a personalized care plan based on their unique needs and preferences. Prior to moving into our community, a licensed nurse will meet with each resident and their family to complete a comprehensive assessment to determine the most appropriate level of support required. Our team will also conduct ongoing care plan meetings to identify any changes in personal care needs, support or medication management. Families take comfort in knowing that we have the ability to care for Supportive Care residents who have more challenging medical needs, which allows delaying or possibly even eliminating the need for a higher level of care. Our residents in this personalized program enjoy private apartments with supportive care as needed.
