Siemon's Heritage Personal Care Center is a place for living. We are a personal care center, and a proud member of the Consulate Health Care family. We have private and semi-private rooms, and can provide accomodations for couples. Siemon's Heritage is located on 20 wooded acres overlooking Lake Somerset with features including an indoor fountain, and relaxing fireplace. A tranquil waterfall and koi pond as well as a large patio are located outside. Our Roam Alert system allows individuals with dementia to remain in our homelike environment. We are Ambassadors of Care.

Consulate Health Care centers offer post-acute services for those patients who no longer require an in-patient hospital stay but who are not ready to go home. Our outcome-oriented rehabilitation therapy programs help patients rebuild the strength, function and mobility needed for a successful return home. Comprehensive rehabilitation services are provided in-house at every Consulate Health Care center.

Rehabilitation services include Physical Therapy, Occupational Therapy and Speech Therapy. Our rehabilitation professionals are licensed therapists who are specially trained to meet the complex needs of patients recovering from a wide range of illnesses or injuries.

In addition to short-term rehabilitation programs for patients recovering from a hospital stay or illness or injury, each of our centers offer quality, long-term, extended-stay elder care services for those individuals for whom living independently at home is no longer possible.

Our centers offer a warm, comfortable, home-like environment where residents receive personalized care and attention. We encourage and assist residents to remain as independent as possible, and we encourage family involvement in the care of their loved one.

Our assisted living centers place special emphasis on providing an atmosphere of respect and friendship where residents can get the help they want, when they need it.
