The Patriot, A Choice Community, is a not-for-profit continuing care retirement community comprised of 100-bed skilled nursing and rehabilitation center, a 68-bed Personal Care home and the security of a Continuing Care Retirement Community (CCRC) in the historic former Somerset Area High School.
Whether you have recently been in the hospital or are recovering from an injury, The Patriot, a Choice Community, has earned a reputation for providing comprehensive skilled nursing for residents with the highest level of specialized rehabilitation. We’re here to help be the next step after a hospital stay to return home safely.
The Patriot offers person-centered care to each resident. All residents and family members are treated as valued, participating members of the transition team assisting with life after discharge.
