At Commonwealth Senior Living at Hillsville, we know that a “one-size-fits-all” approach to care just doesn’t work. Primary Components

• Move-in: Whether moving to our community from home or hospital, we guide our residents through every step of this journey, meeting every need, from physical to emotional.
• Evaluation through life history: Residents shape their own care, so we start by getting to know each as an individual. We conduct a comprehensive interview with family members, asking questions, listening and learning. We consider all aspects of life, including medical, mental, emotional, spiritual and intellectual needs, as well as habits and preferences.
• Resident-centered care plan: Once we have our resident’s life story, we create a customized care plan to meet their specific needs and promote their specific strengths.
