Fountainhead Homes is intimate and tailored care at its finest. Because the details matter, we are committed to serving no more than 8 residents in each of our homes. Our locations are beautifully designed, supremely comfortable, and offer virtually unlimited opportunities for the personalization of resident living spaces.
We welcome residents at all stages of their healthcare journeys. Our low staff-to-resident ratio makes the transition to Fountainhead Homes as smooth and comfortable as possible. Additionally, our experienced nursing staff actively participates in all facets of resident care and develops a close rapport with physicians and health care partners. From the day our residents arrive, they are pampered with care. And that care is never “one-size-fits-all.” We get to know the personal needs and preferences of each of our residents, and then deliver the highest level of care based on that understanding. We find that we learn new things from our residents every day, and this enriches all of our lives.
At Fountainhead Homes, we believe that the first step to providing quality preventative care is to understand and recognize who each of our residents is—as a whole, three-dimensional person. Diagnoses and detailed health histories give us the information we need to assess two of those dimensions. But to truly deliver highly effective, personalized care, we need to know about the “third dimension” as well—that is, what makes each of our residents unique, what makes them tick. And we consider it a top priority to do so.
To develop a care plan that works, we are committed to getting to know the unique physical, emotional, and spiritual needs of our residents. This means getting to know their individual backgrounds and preferences: their lifelong routines, loved ones, work and educational histories, spiritual lives, skills, hobbies, and much more. It also means getting to know the smallest of details, like whether it should be sunny-side-up or hard-boiled, comedy or drama, or jazz, easy listening, or rock-and-roll. It all matters.
At Fountainhead Homes, we appreciate the privilege to serve, and we welcome the opportunity to learn and grow from our residents. Every person’s health journey is different. We want to know how each of our residents thinks and feels, what their preferences are, and where and when certain activities fit into the daily rhythms of their lives.
Since 2000 we have been asked to expand to serve more people in our community. The reputation and successes of our comprehensive care program have been spread widely by the words of happy families and healthcare professionals. Finally in 2015 we opened two of our six CBRF’s at our Savannah Drive location in DeForest at the intersection of highway 51 and highway 19 & easy off Interstate 94. We sit across from the new Meriter and UW Clinics, but also view the tranquil nature of Token Creek Park.
We are delighted to welcome more residents to touch our lives in special ways, and for us to serve them with our unique kind of tenderness. Each of our new homes offer 8 beautiful private rooms with spacious bathrooms, an additional shower room for greater accessibility and assistance, elegant common areas with tall mission-style windows, a floor-to-ceiling stone fireplace to enjoy alone or with others in a group activity …. modern amenities throughout with a feel of “home”.
