Together, Christian Community Homes and Services’ (CCHS) over 35 years of experience in award-winning senior services and 75 years of personal, quality health care delivered by Osceola Medical Center (OMC) form a solid foundation for this campus. Built on the 134-acre campus of OMC, CCH of Osceola consists of a 40-bed Skilled Nursing Facility and a 20-bed Assisted Living Facility, licensed as a Community Based Residential Facility.

At our assisted living residences, we provide a sensible mix of security and independent living. Assistance and companionship are never far, but privacy and independence are always respected.

Christian Community Home of Osceola’s long-term care program is designed for residents whose health conditions are complex and/or medically unstable and who require frequent medical or nursing intervention. We offer our residents many choices when it comes to their daily lives. Should your loved one enter the care of CCH, he or she along with the physicians, care team, and your family will work together to formulate an individualized plan of care. This comprehensive approach is designed to address not only his or her health needs, but functional abilities, chronic illnesses and desired outcomes.

Our therapy gym and wellness center helps bridge the gap between hospitalization and returning home. It offers short-term rehabilitation that enables you to quickly regain your health and strength so you can return home.  The collaboration between your physician, the experienced skilled nursing staff and the rehabilitation staff will help you become as independent as possible.

We offer onsite physical, occupational and speech therapy.  If needed, tut-patient therapy can also be continued after you return home.

The therapists at Christian Community Home work together as part of the interdisciplinary team with nursing, dietary, therapeutic recreation and social services to design therapy programs that facilitate independence for our community residents.
