Ivy Manor Assisted Living Facilities and the Ivy Manor Memory Care Unit, provide state-of-the-art facilities in a warm, cozy, home like setting. We serve the frail and elderly, physically disabled, terminally ill and those suffering from Dementia and Alzheimer’s. Our qualified and dedicated 24 hour staff, assist residents with their day to day physical, medical, mental, nutritional, social and spiritual needs and provide a strong support system for the resident’s families. Maintaining independence is a big deal for people as they age. In fact, losing it is one of the biggest fears seniors have. An overwhelming majority want to remain in their own homes – and understandably so. But the reality is that almost half of U.S. adults age 65+ say they either need or currently receive help with activities of daily living (ADLs). Is there a way to get that kind of help and still be as independent as possible? Yes – it’s called assisted living. Simply put, Ivy Manor Assisted Living can give older adults personalized care in a residential setting. They’re for seniors whose health or well-being requires a higher level of personal care.
Since opening our doors, we have come to find a greater and greater need for specialized care in regards to those with advanced Alzheimer's and dementia. Generally residents with early stage Alzheimer’s or dementia can live among the regular population in assisted living facilities. But when the condition becomes advanced, families were having a hard time finding an appropriate resource to assist their family and loved ones. To better meet the needs of our community, Ivy Manor of West Bend offers a secure & dedicated facility to protect residents with severe memory care needs, and also has staff specially trained to care for those with the disease.
Each resident will receive a Level of Care (LOC) assessment prior to admission and periodically during their stay in order to promote the highest level of independence. Prior to admission to Ivy Manor, each prospective resident will receive an initial assessment determining their initial rate. Within the first thirty (30) days of admission, a detailed Level of Care Assessment (LOC) will be done, determining the continued LOC. The Level of Care plan is designed in a way to encourage residents to function as independently as possible. A general outline of the care levels are as follows:
Level 1: Monitored Care
The Monitored Care level is for the resident who completes their activities of daily living autonomously. Assistance with personal care is not required on a daily basis, but a supportive environment with basic services is provided. Basic service includes:
- Private Room w/Private Bath Medication Monitoring/Administration
- 24 hour supportive environment
- Housekeeping Services
- Health Monitoring
- Laundry Services
- 3 Home-cooked meals a day
- Cable TV in Common areas
- Nutritious Snacks each day
- Utilities
- Leisure time activities
- Local Telephone calls from house phone
- Weekly bathing assistance
Level 2: Supportive Care
The Supportive Care level is for residents who require frequent assistance with most activities of daily living. Along with the basic services listed under level 1, the resident may require assistance with bathing, grooming or personal hygiene. The resident may also require one or more of the following services:
- Incontinence Care- Occasional
- Occasional Redirection and Orientation
- Special Dietary Modifications
- Needs assistance less than daily
- Medication Management
- Assistance with Equipment needs(oxygen, brace, etc)
- Cueing/Supervision for completion of activities of daily living
Level 3: Assisted Care
The Assisted Care level is for residents who require daily assistance from a caregiver to complete their activities of daily living. Along with the basic services in Level 1 & 2, the resident may require one or more following service:
- Assistance Eating
- Specialized 1 to 1 activity time
- Incontinence Care - Daily
- 1 Person Transfer
- Challenging Behavior Management
- Hospice/Palliative Care
- Extensive Medication Management
- Diabetic Care (Accuchecks, Insulin)
- Assistance/Stand-by assist for completion of activities of daily living
Level 4: Extended Care
The Extended Care level is for residents who require daily assistance with their activities of daily living and may have some medically complex issues requiring the RN or LPN more frequently. Along with the basic services offered in level 1, 2 & 3, the resident may require one or more following services:
- 2 Person Transfer
- Use of Hoyer Lift or EZ-stand
- Complex Medication Management
- Use of more than 1 Schedule II pain medications
- Ostomy care
- Catheter care
- Pain Management
- Specialized Dietary Needs
- Wound care/dressing changes daily
