Heart & Soul Communities is an active assisted living community in Oakland, California. It is within bright and cheery Alameda County which is part of the Golden State.

Heart & Soul Communities is a place dedicated to senior service, and where meeting their physical, spiritual, and emotional needs in an environment that recognizes their cultural differences, is a way of life. Heart & Soul is not only committed to the lives of our seniors, but also to the souls of our seniors.

Our culturally competent atmosphere allows each client to be cared for as an individual, with a passion to understand their past experiences, cultural sensitivities and beliefs, values, so that we can integrate our quality care model with their specific needs in mind. 

Heart & Soul Communities consistently raises the bar in resident care! Resident care is our top priority, and we are always looking for ways to improve the care we provide. Among our stellar team of employees, we have a registered nurse who visits each of our facilities monthly to monitor residents weight, blood pressure, heart rate, saturations, skin integrity, mobility, and glucose level (if needed). In addition to this, the nurse checks our Medicine Administration Charts and Client Logs to ensure residents overall conditions are stable.

Food is one of the most important aspects of quality care. Ensuring that the food is nutritious, presented well and delicious is of the utmost importance! Heart & Soul Communities continues to raise the bar to ensure that our residents love the food and feel the love through the food we serve. We continue to extend our food repertoire to better serve our residents and are currently working to expand our garden!

We pride ourselves on the condition of our residents and the condition of our facilities! At Heart & Soul Communities, we make sure our residents are well-groomed, clean, cared for and comfortable. We ensure this by making sure our residents hair/beards are maintained and styled, their clothing neat and clean, and implementing a daily exercise routine. We also like to make sure our facilities are comfortable with adequate seating and lounging space to ensure comfort. We want our residents to feel at home in a place they can be comfortable and enjoy.
