Shortly after admission to our facility, the Interdisciplinary Care Team (IDCT) will meet with you and your family members to develop a Care Plan.  Each discipline (Nursing, Therapy, Nutrition, Activities, Social Service) of the IDCT asks questions to make sure you receive the unique services your individual situation requires.  When a patient isn’t able to effectively communicate, family members are heavily relied upon for key information needed to help the patient feel comfortable and get as well as possible.  A broad spectrum of information about the patient is taken into consideration, from medical data such as diagnosis and prior level of function, to personal information such as likes and dislikes, religious preference, social and family support and personal history and interests.

For those residents needing an extra helping hand with their activities of daily living, our Assisted Living Care Givers provide both full and partial assisted living services. We provide assistance with activities of daily living and the coordination of services by outside healthcare providers to help to ensure health, safety, and well-being.

Our focus is on individual wellness, and we encourage continued independence with a tailored plan of assistance, provided by our well-trained team members who are available around the clock, 24-hours a day. We are at your service!

The therapy staff is part of the “bricks and mortar” of Creekside Transitional Care & Rehabilitation, and our therapists take pride in the performance of the therapy department as well as the entire facility.  We understand each patient has unique and special needs, so our in-house therapy program is completely customizable to meet them. This is a collaborative effort among our team members, who work with your physician to create a unique vision and treatment plan for you. We can even provide specialized equipment to aid in your treatment.

Our approach to therapy is truly state-of-the-art, and we provide the finest in therapists, equipment and services. Our therapy room is large and clean, well-lit and welcoming. We’ll design a therapy program that is tailored to get you up, around, and functioning at the highest level possible. It’s our goal to get you back in the swing of things, and we employ a variety of high-tech and (good old-fashioned) hands-on therapeutic approaches that we believe will work wonders for you!
