Matrix Home Health Care Specialists is a privately owned (not franchised) Care Management and Home Health Care business run by a hands-on nationally certified gerontological nurse who is also a Certified Case Manager. Matrix has been providing trusted quality care in Minnesota for over 25 years.

We are a Comprehensive licensed home care agency providing a continuum of care options from as little as 4 hours once a month up to 24 hours a day 7 days a week. We use a unique team based approach that minimizes the number of caregivers in the home and enhances the client and caregiver experience. All home caregivers are Matrix employees who are carefully screened and fully trained above and beyond what is required by the state. All Matrix home care clients work with one of our RN Care Managers that develops a Plan of Care in conjunction with the client and family, and supervises the home caregivers closely to ensure that homecare services are of the highest quality and provided skillfully, respectfully, and safely.

Our RN Care Managers assess a client’s physical, mental and social needs and work with the client and/or responsible party to develop creative, responsive care plans that maximize each client’s functional status, independence, and dignity. All our RN Care Managers have expertise in geriatrics, nursing and health care issues as well as special skills in assessment, care planning, and knowledge of community resources.

At Grace Homes – Oakridge in Hopkins we offer assisted living services in a sprawling rambler style home with an expansive shaded property and large accessible porch. Residents enjoy activities and independence with the comfort of knowing on-site health care and personal assistance is there when they need it. Oakridge is ideal for people with complex or chronic health needs, memory challenges and those at the end of life. Situated immediately adjacent to our Wilshire Walk home we offer a large activity room, spa room and more nestled in a quiet residential neighborhood surrounded by mature trees and gardens.

Oakridge offers numerous gathering areas for residents to spend time together, with family members and other visitors. Our one level family-style care home accommodates wheelchairs throughout. We have a resident dog and cat as well a other visiting pets at times.

On our expansive deck we offer summer dining and activities. Relaxing with families, visiting with friends and sharing a cool beverage are perfect while outdoors.Sunshine, fresh air and being in nature is refreshing for all.

We strive to promote engagement and activities which include a daily schedule of exercise opportunities individualized to the ability and interest of each person.

Our staff provides support for our residents to pursue those activities that interest them including arts, crafts, reading, gardening and more. We engage performers from throughout the metro area to diversify and expose our residents to a greater variety of music, instrumentals and even Karaoke opportunities.

Oakridge’s spacious indoor and outdoor areas promote room for each person to enjoy time doing what they enjoy most.
