Matrix Home Health Care Specialists is a privately owned (not franchised) Care Management and Home Health Care business run by a hands-on nationally certified gerontological nurse who is also a Certified Case Manager. Matrix has been providing trusted quality care in Minnesota for over 25 years.

We are a Comprehensive licensed home care agency providing a continuum of care options from as little as 4 hours once a month up to 24 hours a day 7 days a week. We use a unique team based approach that minimizes the number of caregivers in the home and enhances the client and caregiver experience. All home caregivers are Matrix employees who are carefully screened and fully trained above and beyond what is required by the state. All Matrix home care clients work with one of our RN Care Managers that develops a Plan of Care in conjunction with the client and family, and supervises the home caregivers closely to ensure that homecare services are of the highest quality and provided skillfully, respectfully, and safely.

Our RN Care Managers assess a client’s physical, mental and social needs and work with the client and/or responsible party to develop creative, responsive care plans that maximize each client’s functional status, independence, and dignity. All our RN Care Managers have expertise in geriatrics, nursing and health care issues as well as special skills in assessment, care planning, and knowledge of community resources.

At Grace Homes – Wilshire Walk in Hopkins we offer a bright atmosphere in a spacious rambler home featuring a large front yard overlooking a nearby pond. Wilshire is ideal for people with complex or chronic health needs, cognitive challenges and those needing care through the end of life. This rambler home located next door to Grace Homes – Oakridge has a bright full glass walled living room, open concept dining area with a large picture window and an aviary. A nicely shaded porch is perfect for warm weather enjoyment. The aromas of fresh home cooked meals are abundant when our personal cook is preparing each day’s personalized fare.

Sunlight and brightness soak every room in Wilshire gathering areas. Our residents enjoy their community spaces and ‘favorite’ chairs to meet for meals, movie time or sing along with performers.

Providing customized living and personalized care to enhance each person’s quality of life. Residents rooms are their own to personalize, making the adjustment to residential living easier.

We promote independence and encourage participation in daily home activities while having options for safety and monitoring for those who are at risk for falls and other concerns.

A residential care home is a great choice for a long-term residence for those needing a hand with daily living needs. Our 1:3 caregiver ratio allows each resident the time and assistance they require to function at their very best.

We work with the many supportive services our residents require including Day Programs, Restorative Therapies, Social and Spiritual care support and many others.

Residents are encouraged to participate in familiar daily activities including assisting with chores, folding laundry, making their beds, clearing their dishes and whatever provides purpose and uses their skills and talents.

Families are always welcome and our doors open to those who provide support to our residents.
