Gianna Homes Sursum Corda

Gianna Homes – Sursum Corda is a large walk-out rambler style home located on a one and one-half acre wooded lot in a quiet residential neighborhood of Minnetonka. All of our resident rooms are on the main level where we also have four (4) bathrooms (two being very large with a full handicap shower), a kitchen, a dining room, a four season porch with views of the woods, a parlor, solarium, a living room with fireplace, and an elevator which takes residents to the lower level. The lower level features a large family room (where we hold activities), a chapel, a laundry room, a carport (for covered access), our offices, and an additional bathroom. The lower level opens out to a beautiful, fenced patio and garden area with walkways, a fountain, and abundant wildlife. There is a generous amount of natural light, as well as artificial lighting to maintain the safety of our residents as they age and sight and gait diminish. There are handrails throughout the home. The home is modestly but tastefully decorated to promote independence and comfort.

In our person-centered approach to memory care, meaningful life engagement opportunities become the essence of the lives of cognitively impaired people. The program we provide at Gianna Homes changes the experience with Alzheimer’s disease from a “long-goodbye” to a positive life experience! Our approach to care is designed so that our residents experience life. Three elements of our program make this happen.

The Lifecare® Services of Gianna Homes are meant to provide not just physical care, but also emotional and spiritual care services for every individual. Gianna Homes aims to foster a sense of a personal wellbeing and independence while facilitating day-to-day activities and addressing individual needs. Our goal is to make the end-of-life experience a positive and worthwhile one to those in our care.

Our Lifecare® Services are numerous and comprehensive, covering all areas of life and the different aspects of individual needs. Memory loss can be one of the most difficult challenges of life, but it doesn’t have to be a lonely one. Gianna Homes is here for all you memory care needs. Welcome home!

It is our goal to offer a variety of activities that address and validate the social, sensory, emotional, spiritual, and physical needs of our residents. Programming is structured around the residents’ interest and abilities so that residents can truly participate and enjoy the day’s events.

We believe activities that revolve around the daily jobs and duties that one would do at home can enrich a resident’s quality of life.

We strive to encourage residents to participate in their own personal cares, as well as share in some of the cooking, cleaning, laundry, and other daily activities of our household.

Our Life Engagement Coordinator, our staff, volunteers and a variety of additional skilled professionals make up our life engagement program to provide meaningful activities to our residents. These activities and programs vary from group and individual music therapy sessions, massage therapy, physical and occupational group sessions, individual therapeutic art sessions, sensory activities, exercises, walks, gardening, picking flowers from the garden, outings, reading and much more. Volunteers provide our residents with therapeutic pet visits, intergenerational activities as well as musical performances.

The residents at Gianna Homes have several opportunities throughout the day to engage in structured activities, that being said, we are not a nursing home. We are a residential home and our home environment is set up to reflect just that.
