Crystal Oaks Assisted Living and Skilled Nursing were established by Jefferson Memorial Hospital in 1995 and 1997, with the mission of expanding their high quality healthcare services to the local community.
In 2007, the hospital created Crystal Oaks Long Term Care, and transferred their assisted living and skilled nursing operations to this independent corporation. Crystal Oaks purchased the existing complex from the hospital in 2008, and expanded their services to include short term rehabilitation and respite care at The Oaks Rehabilitation Center.
Crystal Oaks offers assisted living, skilled nursing, inpatient and outpatient rehabilitation and all levels of memory care on our beautiful, wooded campus just south of Mercy Hospital Jefferson. We serve the needs of many seniors as they transition through the various levels of care that our campus has to offer. Our location provides easy access to doctor’s appointments and medical care and our services and amenities provide all of the luxuries and comforts of home.
Crystal Oaks Assisted Living is the ideal solution for seniors who value their independence, yet require assistance with daily activities (such as bathing, dressing and dispensing their medications). Individual wellness and assistance plans are carefully coordinated by our staff, ensuring the comfort and confidence of each one of our residents.
Assisted Living has nursing staff available 24 hours a day, seven days a week. Residents will enjoy private rooms which encourage continued independence, and include:

Our focus is to provide stability, familiarity and reassurance for our residents, while allowing them the greatest degree of independence as possible. Our structured programs and dedicated staff promote an environment of support and safety, which helps each resident make the most out of every day.
We have developed two innovative levels of care to address the specific needs of those with memory loss: ALDEN PLACE and SAVANNAH PLACE. Located in our Assisted Living and Skilled Nursing settings, each unit is designed to meet the appropriate level of care for our residents.
Memory care and support is delivered in a very personal manner at Crystal Oaks. Those living in ALDEN PLACE and SAVANNAH PLACE have unique challenges and preferences; and we recognize that the care required to meet their needs changes over time. An individual plan of care is developed for each resident after they move in, and is updated periodically to ensure that an appropriate level of assistance is being provided at all times. Care plans will address specific needs in the areas of bathing and personal hygiene, dressing and grooming, mobility, socialization, and eating.
Crystal Oaks Skilled Nursing services are designed to provide full-time nursing and medical services to seniors with more serious or complex health needs. The commitment and devotion offered by our therapists, staff and volunteers provide each resident with the highest level of personal care, a sense of true companionship and a feeling of genuine warmth.
Skilled Care offers private and semi-private rooms, and the following medical services:
- Medical director on staff
- Physical, occupational and speech therapies on site
- Registered nurse on site 24 hours a day
- Medication management and collaboration with physicians
- Admissions seven days/week
- Medicare, Medicaid and private insurance accepted
For shorter term stays and recovery from surgery, stroke or other injury, we provide an ideal environment for healing and recovery at The Oaks Rehabilitation Center. This facility is located on the Crystal Oaks campus, adjacent to the skilled nursing facility.
