Hopeton Village is now accepting applications for Assisted Living. Assisted Living will include an apartment, personal care services, meals, emergency response systems, and medication administration.
The Assisted Living Waiver Program pays the costs of care in an Assisted Living facility. Hopeton Village is a rent-subsidized community and the consumer is responsible for “room and board” expenses. The program is open to individuals currently enrolled on another home and community-based waiver program such as PASSPORT, Choices, or the Ohio Home Care waiver or residing in a nursing home. Participants must be financially eligible for Medicaid and determined by Area Agency on Aging (AAA) to need a “nursing facility level of care,” the same requirements as PASSPORT program.
