Sylvan Crossings homes are licensed in Wisconsin and located throughout the state. We offer assisted living, short-term stays after hospitalization or prior to deciding to move here, respite care, and hospice care. Some homes have an on-site rehabilitation clinic. Wellness is one of our focus points and keeping residents as independent as possible is one of our goals.

Residents receive whatever type of assistance they need, such as bathing, grooming, dressing and medication reminders or assistance. Our home is located in Fitchburg, just minutes away from Madison’s beautiful State Capitol. It’s an ideal location for those who appreciate quiet living, but enjoy being near shopping, medical facilities and restaurants.

Personal, dietary, medication, and life enhancement services are tailored to a resident’s needs. Those needs may change from time to time, depending on the resident’s needs.

When a resident chooses to make Sylvan Crossings their home, they meet with an interdisciplinary team that includes their families. Together we assess the services that are needed.

​Residents are encouraged to bring their own furniture so they feel more at home. Each suite is spacious, has a 24-hour monitoring system, a full bath, individual thermostat, and an emergency call system.

​Most residents appreciate routine assistance with personal care, such as bathing, medication monitoring, and dressing. Residents with walkers or wheelchairs enjoy mobility assistance. Incontinence care promotes independence and helps retain dignity. Cueing assistance for memory loss is important for many people. Optional services include on-site hair and nail care and assistance with transportation to offsite locations.
