Our dedicated and professional staff in combination with physicians will perform comprehensive assessments to accurately determine the level of assistance needed for personal care, ambulation, and activities of daily living, such as: bathing, dressing, grooming, and meal preparation.
All comprehensive assessments are person-centered. We focus on evaluating each resident health, mental and physical status and limitations to determine each resident need. We will develop a plan of care guaranteeing that each resident receives the level of care, supervision and assistance 24/7 require.
The facilities goal is that our personalized approach helps our residents maintain the best quality of life, to keep them socially engage, and to improve their confidence and self-esteem by having programmed community activities and wellness program. We value the uniqueness of each resident, promotes engagement and provides a sense of purpose.
We will provide housing, meals, and one or more personal services for a period exceeding 24 hours to one or more adults who are not relatives of the owner or administrator; residents will reside in a safe, comfortable and sanitary sleeping environments; housekeeping services, cleaning of resident’s room; and the provision of all linens, towels, soap, and toilet paper are included. Also included are the following services:
- Supervision of self-administration of medication
- Assist with Activities of Daily Living (ADL’s) such as: Ambulation, Bathing, Dressing, Eating, Self Care (grooming), Toileting and Transferring
- Housekeeping (including laundry)
- Mental and physical stimulation (social activities)
- Three meals and snacks, guided by a dietitian/nutritionist and health care provider

We will have trained staff in the (8) Hrs. Limited Mental Health Training conducted by Department of Children & Families. Then every two years staff will maintain an additional (3) hrs. Training Update.
We will make sure to have a Community Living Support Plan & Cooperative Agreement for the LMH Resident from their Psychiatrist to be able to follow the care plan to try to help avoid an unnecessary crisis.
We upon determination by the administrator or health care provider that the resident needs services beyond those the facility is licensed to provide, the resident or next of kin, legal representative, or the agency acting on the resident’s behalf, shall be notified in writing that the resident must make arrangements for transfer to care setting that has the services needed by the resident. In the event a resident has no person to represent him, the facility shall be responsible for making a referral to an appropriate social service agency for placement.
